The first two days of the meeting, hosted by the Polish Society, included topics on breast reconstruction, treatment of lymphatic oedema and breast enlargement. A great lecture was given by prof. Aneta Undas on thromboembolic complications following plastic surgery operations. This very important topic concerns the application of practical prevention measures, so-called thromboprevention. The lectures were great, several very important issues were clarified in terms of the risk of thrombolic complications in plastic surgery.

The last day hosted the ISAPS Symposium ‘Aesthetic Surgery of the Face’. We had the great pleasure of hosting prof. Nazim Cerkes of Turkey, a world-class plastic surgeon executing the most complicated nose operations. The afternoon session dealt with operations of the face and neck. Two excellent surgeons, Dr Gianloca Campiglio of Italy and Dr Apostolos Mandrekas of Greece presented their work, effects, achievements. It was a great pleasure to host such eminent doctors at a meeting of the Polish Society of Plastic, Reconstructive and Aesthetic Surgery.

The evening social meetings were an opportunity to talk privately and make new friendships.

June 7th and 8th saw Verona hosting the International Face and Nose Anatomy Course „Beauty of the face”. The course was organised for 15 persons, the lecturers were: Dr Natale Gontijo de Amorim, Dr Riccardo Mazzola, Dr Gino Rigotti, Dr Enrico Robotti, Dr Andrea Sbarbati, Dr Peter Palhazi.

The course was composed of a theoretical and a practical part. Each of the participants was able to independently conduct cadaver exercises. Dr Peter Palhazi turned out to be an incredible lecturer; he showed us new anatomy of the nose, never described and yet studied by him. Why is this so important? Because, until now, nobody has found and described such new components of the anatomy of the nose, the knowledge of which has enormous influence on its functions and looks following an operation. I was able to spend an entire day with Dr Palhazi, who was eager to show, prepare and explain, and was keen to share his knowledge on new nose operations. This incredible young man, a passionate surgeon, inscribed his achievements in the history of plastic surgery. I believe that the enormity of knowledge on anatomy, face and nose surgery is so great that it awes on the one hand, and on the other – allows one to use and mix techniques in an almost limitless way. As usual, I recommend participation in such a meeting to all my colleagues.

The main topic were new face, neck, upper and lower eyelid operations.

One could think whether after so many years of face and neck operations, patients may be offered new operations beside new techniques. Well, if the world would still have such great and marvellous surgeons as prof. Giovanni Botti, prof. Mario Ceravolo Pelle, prof. Bryan Mendelson, who eagerly share their enormous knowledge and suggest new operation techniques, then one might say that this is possible. This year, the Gardone meeting was divided into two parts. The first were a cadaver course hosted by these wonderful surgeons, during which the participants were able to try out all the new techniques. The remaining two days were lectures, video sessions and round-table talks on the head, face and eyelids. It is incredible, how many new ideas emerged during the meeting.

Everything that happened this year in Gardone expands very much upon the possibilities of operations. In addition, each day, operations were transmitted live from the private plastic surgery clinic of prof. Giovanni Botti. I think that it is only after returning home over the course of the next few days one can slowly analyse the multitude of excellent and valuable things that happened during this trip. The form of the congress was great, because it gave the opportunity to view operations, participate in cadaver exercises and engage in personal contact with the great ones of this world. The scientific committee included not only the great surgeons mentioned earlier, but also Dr André Auersvald of Brazil (incredible two face and neck operations using the new techniques), Dr N. Joshi (new techniques of eye lid operations), Dr N. Waterhouse (new neck operations), Dr P. Tonnard (new methods of application of fat just after a concluded face and neck operation). Knowing the value of this meeting, to all my colleagues – I greatly recommend participation.

The message of the course was to show many primary and secondary techniques of nose operations. Dr Robotti showed six different, uniquely difficult nose operations. The low number of participants allowed every guest to ask questions to the operating surgeon. Dr Robotti stressed, how difficult and responsible nasal surgery is, how difficult it is to estimate the final effect, the more so that the nose healing process after the operation is very long, and the operator has no influence on the course of the healing. Dr Robotti showed several phenomenal operation techniques that are smart and useful, which I will certainly use in my practice.

Plastic operations of the nose are exceedingly difficult, yet at the same time – fascinating. There are supporters of open-type nose operations, and there are supporters for closed-type nose operations. The organisers of the Congress concluded that the two different views will be the perfect way to provoke a discussion on which method is better. Over two days, eight patients were operated on – four pairs of very similar cases. Each of the surgeons defended their concept of the operation. The world class team of plastic surgeons specialising in face operations, experienced in rhinoplasty, presented advanced methods of nose operations with the use of open and closed rhinoplasty techniques. The course was organised by Dr Sherrell J. Aston, Dr Daniel C. Baker and Dr Dean M. Toriumi. The topics that were discussed in detail covered the aesthetic smoothing of the nose, the formation and correction of the shape of the nasal tip, an enlargement or reduction of the protrusion of the nasal tip, reduction and extension, correction of the form of the nasal wings and the nasal septum, reduction and enlargement of the nasal bridge, nasal functions, corrections of a crooked nose, second- and third-order rhinoplasty as well as variations in rhinoplasty stemming from ethnic and anatomical differences. Fillers and fat are being ever more frequently used to alter the nose. This is knowledge that is necessary for all surgeons executing nose operations.

Forty-three five-minute films and six longer interactive films transported the viewers to the operating room, where experts presented detailed nose formation techniques. The films concerned themselves with the following aspects:

– individual techniques of smoothing out of the nose profile and ostheotomy,

– control of the protrusion and length of the nasal tip,

– deformations of a crooked and a saddle nose

– handling the base of the nasal holes

– enlargement of the ridge: how to go about it?

– precision and detail in the first open rhinoplasty,

– reconstructive aesthetic rhinoplasty

– transplantation using rib cartilage

– extrasystemic septoplasty

During the course, all lecturers defined nose operations as the plastic surgery operations that are most difficult from the technical point of view. They stressed that the final effect is unforeseeable, and that it may give rise to patient dissatisfaction. Dr Nazim Cerkes concluded that nasal operations teach plastic surgeons humility stemming from the level of difficulty of nose operations and the expectations of patients. A plastic surgeon commencing a nose operation must:

– define the roles of fillers and fats, as they are related to the aesthetics of the nose;

– estimate the anatomic basis for the recognised nose deformations;

– discuss the advantages, shortcomings, complications and long-term results of closed rhinoplasty,

The conference is always composed of several parts. The first part are live operations, followed by video sessions and round-table talks. Over the course of several years, the best surgeons in the world were presented, the most modern operations were shown. The topic of the last meeting were treatments in the area of bariatric surgery, meaning, operations on patients who were significantly overweight.

The shaping of the body has the purpose of modification of various parts of the body following significant weight loss. This refers to a series of procedures designed to reduce the excess volume of flat skin and fat in people who were formerly obese. The live operations showed brachioplasty, internal and horizontal lifting of the upper legs, horizontal lifting of the buttocks, abdominoplasty and circumferential body shaping.

Particular attention was given to the anatomy of the lymphatic system, to anatomy of the skin and subcutaneous tissue as well as the tension level of the named tissues. The lecturer was the famed French plastic surgeon, creator of the French school of postbariatric surgery, Dr Claude Le Louarn, student of Paul Tessier and Claude Dufourmental, head of the SOFCEP (the French Plastic Surgery Association).

The meetings took place in the wonderful rooms of the Le Plaza hotel. Inspired by the Hotel George V of Paris, the Le Plaza remains a lasting source of awe. Its unique theatre, originally a movie theatre named just Le Plaza seats a stunning 1300 guests. Since 1922, the hall and the lobby are inscribed in the list of Belgian national heritage thanks to their architecture and design. This was also a unique occasion to see Brussels, the heart of Europe, with its golden market square, narrow, mediaeval streets, old antiquarian stores, well-known restaurants as well as famous Art Deco and modern art museums.

The meeting was divided into two parts. The first part dealt with lectures concerning new breast implants that emerged two years ago. During the lectures, the advantages of silicone gel and the implant coatings were presented, with particular focus on the size of the silicone particles. Over the course of two years, twelve thousand implants were implanted, with no complications recorded, and not a single case of connective tissue pouch.

The lectures were held by scientists from the laboratory of the company manufacturing these implants.

The second part was dedicated to topics concerned with immunology and bacteriology of the pouch, into which the implant is placed. Uniquely interesting studies have been conducted in the United States and Europe for eight years now. For those interested in rhinoplasty at any stage of their careers, this symposium was a unique opportunity to dive into the topic, a chance that might not repeat itself for years now. Irrespective of the location of the symposium, the sheer educational quality coupled with the experience gained made this conference into an event that simply could not be missed. The symposium took place in the Versailles palace, a place that is uniquely prestigious and fitting in perfectly with the significance of the conference.

As always, together with the Villa Bella surgeons, well-known plastic surgeons from all over the world showed the course participants the newest achievements of their treatment techniques, sharing also their most recent discoveries, hints and tricks.

Discussion panels touched upon controversial topics, limitations related to techniques and procedures as well as complications and patient safety.

The first session concerned itself with operations of breasts. Discussed were the modes of simultaneous enlargement of breasts along with their suspension. Handled were problems and strategies of operations, complications and the possibility of their removal. One of the topics were secondary operations entailing inspections of the glands and the points of implantation of the implants. A further topic was breast enlargement surgery in a situation of breast cage deformations. New criteria were described of the measurement and choice of the pouch under the breast muscles. Discussed were the modes of operation of tubular breasts and complications following these types of operations.

The next session dealt with operations of the face – discussed were strategies of procedure in operations of the face and neck, the role of the fat autotransplantation in face operations. An important topic of the session were operations of eyelids. The lecturers provided advice as to how to avoid complications in eyelid operations and how to perform reconstructive surgery following failed treatments.

The last session concerned aesthetic medicine with particular attention to the usage of hyaluronic acids, numerous complications in the form of blood vessel blockage due to the introduction of hyaluronic acid into soft tissue. In course of the lecture, Dr Delorenzi from Canada described grave cases of blood vessel blockage, he showed and explained the newest modes of procedure with patients. He stressed that the leading factors in this regard are time and doses of the drug that must be introduced to the blockage point. Last year, in Canada, at the clinic of Dr Delorenzi, such treatment was used by over 400 patients. The newest method published by Dr Delorenzi is uniquely effective; in emergency cases, it can save the patients’ image, and in grave cases it can also save their lives.

The International Society of Plastic Surgery, ISAPS, takes its members from among plastic surgeons from all over the world. Any doctor who is a member of the society can participate in meetings, congresses and exercises organised all over the world. One does not need special invitations for one, two or several plastic surgeons. The knowledge is generally available and all members feel distinguished by the participation in the meetings. This symposium was attended by two Polish plastic surgeons, one of them was Dr Andrzej Dmytrzak.

The topic of discussions were exclusively rescue procedures following failed aesthetic surgery operations. The topics covered breast, face, eye lid, nose, body and upper leg treatments, and were presented by eminent lecturers.

The live operations was carried out at the Frauenklinik of Dr Geisenhofer and transmitted directly to the hall at the Sofitel. An ambitious operation programme was planned. Some treatments were watched live, others as edited videos. Such a form allowed the best opportunities to interact and learn.

It was the intention of the SOS Symposium to learn from each other mutually and not to blame the surgeon for any complication. We all experience complications. Some complications cannot be foreseen and we are practically forced to encounter them. Complications may also arise due to one’s wrong surgical diagnosis, as a result of wrong surgical skills, wrong choice of the patient or the surgical technique. It was the objective of the symposium to provide solutions to failed cases, and to learn the methods of avoidance of complications in the future. The operation day was followed by the lecture day, with lectures amended by panel discussions.

The lecturers noted legal aspects concerning aesthetic surgery of reproductive organs. Particularly stressed was the operational technique and post-operational complications. Stressed was the significance of aligning recommendations to the treatment. This course was attended by Dr Andrzej Dmytrzak.

Proponents of ‘open nose’ operations were Nazim Cerkes (Istanbul), Olivier Gerbault (Paris), Wolfgang Gubisch (Stuttgart), José Palacin (Barcelona).

The ‘closed nose’ method was defended by Giovanni Botti (Salò), Pietro Palma (Milan), Mario Pelle (Rome) and Alberto Scattolin (Vittorio Veneto). Lectures and video sessions took place, and operations were transmitted live from the Villa Bella clinic of professor Giovanni Botti. Many new post-traumatic nasal operation techniques were shown, as well as nasal operations after failed operations and inborn nose flaws. All lecturers concluded unanimously that nose operations are the technically most difficult operations in aesthetic surgery. The contemporary effect of nose operations is a new structure of the nose, based on transplants of cartilage taken in most cases from ribs or the ear lobe. The congress was attended by sixty plastic surgeons and laryngologists from all over the world, with the sole Polish participant being Dr Andrzej Dmytrzak.

Between June 19th and 21st of this year, Istanbul hosted the 5th international Euro-Asian Plastic Surgery Meeting organised by the ISAPS, and dedicated to the newest techniques in operations: of the face, of the breast (enlargement and reduction), of the belly, body contouring, fat transplants.

The lecturers were Renato Saltz, USA; Elizabeth Hall-Findlay, Canada; Joseph Hunstad, USA; Glenn W. Jelks, USA; Timothy Marten, USA.

On June 20th, Dr Eric Auclair, head of the SOFCEP, the French Association for Aesthetic and Plastic Surgery, had a lecture on new methods of execution of breast enlargement operations.

Dr Andrzej Dmytrzak has been friends with Dr Eric Auclair for twenty years now.

The following two days included the Course and Meeting of the European Nose Surgery Association, during which difficult cases of noses were analysed that were qualified for operations; furthermore, operations as well as new nose operation techniques were discussed. During the course, live operation sessions also took place.

Article 5th International Eurasian Aesthetic Plastic Surgery Course – ISAPS Istanbul Course & Istanbul Advanced Rhinoplasty Course & 2nd Annual Meeting of the Rhinoplasty Society of Europe comes from the site Plastic surgery clinic - Aesthetic Med Dr Andrzej Dmytrzak.

The congress, organised by Dr Giovanni Botti was dedicated to the newest methods, techniques of face and neck operations. The plastic surgeons attending the congress were able to participate daily in several face and neck operations taking place at the ‘Villa Bella’ Clinic headed by Dr Giovanni Botti.

The course topic were latest techniques of operations of upper and lower eye lids, together with operations of the temple and middle areas of the face. For such operations, it is very important to prepare the tissue very well. This method of conducting the treatments allows one to greatly improve the effect of the operations, without the necessity to operate on the entire face. The fat tissue is transplanted at the same time.

All cases of the operated patients were thoroughly analysed in terms of age, skin quality and operation capabilities. Risks of complications and the methods of handling them were also discussed on a continuous basis.

A part of the meeting was dedicated to the newest techniques and methods of operating on the entire face and neck. Dr Giovanni Botti and Dr Mario Pelle presented some of the most modern operation techniques that included fat transplants and muscle modelling. Particular attention was turned to the anatomy of the nerves, facial and neck muscles.

The significance of intelligent simultaneous usage of botox and hyaluronic acid was stressed. Criteria were discussed of the provision of fat or hyaluronic acid.

A low-invasion surgery method was shown to smooth out the horizontal wrinkles of the forehead, and a low-invasive method of lifting the tips of the mouth.
